Proposal Submission Form

    Purpose

    This form is intended to capture basic and necessary information about the proposal in one form. We encourage anyone with a good idea to use this form to start the process.

    The Process

    When finished filling ALL fields on this form, please send it in its entirety (including all pages) to ACCFacilitators@GoogleGroups.com. The Facilitators group will review it and reply to the proposal owner with feedback about the contents. If any fields are missing, the proposal owner will be asked to fill them and resubmit.

    Once the form is complete, it will be scheduled to be presented before the ACC Board of Directors/Executive Committee in the next meeting or in subsequent meetings depending on availability. The Proposal Owner will be informed of the date of the meeting and must be present at that meeting to present the idea in front of the committee.

    The Board of Directors/Executive Committee will either accept or reject or propose modifications and recommendations back to the proposal owner right in the meeting.

    On the following pages, you will have a chance to describe your idea. Please be concise and to the point. This is not a detailed plan for your idea. The detailed plan will come after the basic idea gets approved by the committee. Make sure you answer all questions on the form or risk the possibility of rejection.
